Patient Forms for Your First Visit
There are three forms for new patients: a demographic form, a hearing questionnaire, and the Tinnitus Handicap Inventory if ringing is part of what brought you in. Arrive about fifteen minutes early and we will have them ready at the desk.
Bring any previous hearing test results and your current hearing aids, if you have them. Old audiograms tell us whether your hearing has held steady or slipped, and we can check the devices while you are here.
